Special Olympics Montana
Distinguished Service Awards
Nomination Form

Use this form to nominate someone for NEXT year!
It is never too early to recognize outstanding contributions to the Special Olympics movement.

Nomination Guidelines

Distinguished Service Awards given by Special Olympics Montana provide a unique and important opportunity to recognize outstanding members of the SOMT community. Anyone who has participated in Special Olympics Montana is eligible for consideration. The judges look for those nominees who have made a significant impact on the quality of SOMT. Award recipients are recognized at the Distinguished Service Awards Celebration to be held in Great Falls, usually the last Saturday in September.  (See description of Categories.)

Rules

Those submitting nominations are encouraged to include a detailed narrative describing the nominee's distinguished service and length of personal commitment to SOMT. Narratives should be limited to ONE typewritten page.  SOMT encourages supporting documentation but will accept only two additional pieces not to exceed one typewritten page each.

Supporting documents may take the form of letters of recommendation, newspaper clippings, or photographs but only two, in addition to the one-page narrative, will be accepted.  Videotapes will be accepted but must be cued and limited to eight minutes.  Nomination deadline for Athlete of the Year is July 9, 2010.

Anyone may submit a nomination and there is no limit to the number of nominations you may submit.
